The IRS has posted a draft version of the form to be used for FATCA registration.

Form 8957 [PDF 136 KB] Foreign Account Tax Compliance Act (FATCA) Registration, has a “watermark” date of May 25, 2018. 

The draft FATCA registration form includes a statement that it is not to be relied upon for filing purposes, and is subject to change and to OMB approval before being officially released.
